We participate with most insurance companies. Please contact our office and your insurance company to confirm that we are an approved provider.

Please be prepared to present your current insurance card and a photo ID at every appointment. For all HMO insurances with primary care physicians, our doctor's name must appear on the insurance card as the PCP or we may have to reschedule your appointment.

Co-payments must be paid at the time of your appointment. Our office accepts cash and major credit cards. Checks are not accepted.

Coordination of Benefits

Coordination of benefits (COB) helps ensure that members covered by more than one plan receive the benefits to which they are entitled while avoiding overpayment by either plan.

Prior to processing claims most insurance companies require that members contact customer services to update information, especially when you are covered under another plan, when your other coverage changes or when your other coverage cancels. Customer services can be reached at the number listed on your insurance ID card.


If you haven't filled out a patient demographic in the past six months, you can shorten your wait at the office by printing and filling it out ahead of time.
